Behavioral Health Services | Care Coordination

Care Coordination Program

In partnership with the Illinois Department of Healthcare and Human Services, CYFS provides intensive care coordination and wraparound services for Medicaid enrolled children under the age of 21 who have complex behavioral health needs and require intensive home and community-based services. Individual Support Services Funds and Therapeutic Support Services allow us the opportunity to access services such as sports, classes, and therapeutic services not covered by Medicaid.

Care Coordination provides intensive case management to youth and families by:

  • Identifying and addressing unmet needs
  • Recognizing and developing strengths
  • Building familial and natural supports for sustainability
  • Coordinating and communicating across community systems
  • Developing skills with youth & family through team-based approach

Components of a Care Coordination Plan

Every Care Coordination Plan includes four components.

  • Strengths, Needs & Cultural Discovery: This community and empowerment tool is family-driven. It provides a deeper understanding and identifies underlying needs.
  • Child & Family Team Meeting: The plan includes regularly scheduled meetings for families to set goals and have their needs addressed and their voices heard. These meetings include formal and informal support and ensure the family is moving through the care coordination process.
  • Crisis Prevention & Safety Plan: In collaboration with the family, we’ll develop an individualized plan to develop a guide for managing and preventing crises.
  • Transition Plan: We’ll provide a transition plan to prepare families to succeed after services. It will identify continued needs and set attainable goals.

Phases of the Care Coordination Program

Engagement Phase

  • Build relationships and establish trust
  • Explain the Wraparound process
  • Prepare youth and family for initial Child & Family Team Meeting

Planning Phase

  • Facilitate initial Child & Family Team Meeting
  • Build and plan for supports and services
  • Develop team relationships and trust

Implementation Phase

  • Provide ongoing support, interventions, and assistance
  • Strengthen team relationships and build natural supports

Transition Phase

  • Finalize preparation for the family to transition out of formal Wraparound
